ComForCare Home Health Care - Northern CO in Fort Collins, CO is seeking a compassionate RN Home Care Case Manager to join our growing team. You will conduct comprehensive assessments, develop personalized care plans, and coordinate care across a multidisciplinary team in patients’ homes.

Qualifications

• Active RN license in Colorado (or compact license eligibility).
• Home health, case management, or community health nursing experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, organization, and time management abilities.
• Proficiency with electronic health records (EHR) and clinical documentation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans for home health clients.
• Coordinate and manage all aspects of patient care, including scheduling, referrals, and interdisciplinary team communication.
• Perform skilled nursing visits, monitor patient progress, and adjust care plans as clinically indicated.
• Educate patients and family members on diagnoses, medications, and self-care techniques.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure continuity of care.
• Maintain accurate and timely clinical documentation in compliance with regulatory and agency standards.
• Ensure patient safety and advocate for optimal health outcomes throughout the care continuum.
