Job summary

BrightStar Care of Chicago and La Grange is seeking an experienced RN Case Manager to join our clinical team. This role is primarily field-based in the Chicago area with regular office time at 222 S.

Morgan St., and extensive daily local travel (~80% field). You will conduct patient assessments and re-assessments, manage an active caseload, and serve as a clinical resource for field nurses, coordinating care plans and ensuring regulatory compliance.

Qualifications

• Active, unrestricted RN license in Illinois
• Critical care clinical experience
• Reliable vehicle with valid driver's license and auto insurance
• Comfortable with extensive daily driving across the Chicago area
• Negative TB test and Physical
• Current CPR certification
• Strong assessment and clinical decision-making skills
• Excellent communication skills for clinical resource support
• Proficiency with electronic documentation systems

Responsibilities

• Conduct comprehensive new patient assessments in clients' homes
• Perform scheduled and ad-hoc re-assessments to evaluate changes in care needs
• Manage an active caseload and coordinate care plans
• Serve as clinical point of contact for field nurses and guide on complex cases
• Communicate with physicians, families, and team members about status and care plan changes
• Ensure care plans and documentation meet regulatory standards
• Identify and escalate clinical concerns appropriately
• Participate in office-based meetings and case conferences (~1 day/week)

Pay: $90,000 Annually

Position Summary

We're looking for an experienced RN Case Manager to join our clinical team. This role is primarily field-based, conducting patient assessments and re-assessments in clients' homes throughout Chicago, while managing an active caseload and serving as a clinical resource for our field nursing staff. The ideal candidate combines strong critical care clinical judgment with the flexibility and independence needed to manage a mobile, client-facing schedule.

Key Responsibilities

• Conduct comprehensive new patient (start of care) assessments in clients' homes
• Perform scheduled and as-needed re-assessments to evaluate changes in patient condition and care needs
• Manage an active caseload, coordinating care plans, updating documentation, and ensuring continuity of care
• Serve as a clinical point of contact for field nurses, answering questions and providing guidance on complex cases
• Communicate with physicians, families, and interdisciplinary team members regarding patient status and care plan changes
• Ensure care plans and documentation meet regulatory and Joint Commission compliance standards
• Identify and upscale clinical concerns or changes in condition appropriately
• Participate in office-based team meetings, case conferences, and administrative work (~1 day/week equivalent)

Qualifications

Required:

• Active, unrestricted RN license in the state of Illinois
• Critical care clinical experience
• Reliable personal vehicle, valid driver's license, and current auto insurance
• Comfortable with extensive daily driving across the greater Chicago area
• Negative TB Test and Physical
• Current CPR Certification
• Strong assessment, critical thinking, and clinical decision-making skills
• Excellent communication skills — comfortable being a clinical resource for peers
• Proficiency with electronic documentation systems

Preferred:

• Minimum 1 year of ICU experience
• Home care, hospice, or case management experience
