BrightStar Care of Chicago is seeking an experienced RN Case Manager to join our clinical team. This field‑based role conducts patient assessments and re‑assessments in clients' homes across Chicago, manages an active caseload, and serves as a clinical resource for field nurses.

The ideal candidate has an active Illinois RN license, critical care experience, strong clinical judgment, and excellent communication skills.

Formación

• Active Illinois RN license required.
• Critical care clinical experience required.
• Strong assessment, critical thinking and clinical decision‑making skills.
• Excellent communication and teamwork abilities.
• Proficiency with electronic documentation systems (EMR/EHR).
• Reliable personal vehicle and valid driver’s license.

Responsabilidades

• Conduct comprehensive start of care assessments in clients' homes.
• Perform scheduled and as‑needed re‑assessments to monitor changes.
• Manage an active caseload and coordinate care plans.
• Serve as a clinical resource for field nurses.
• Communicate with physicians, families, and the interdisciplinary team.
• Ensure care plans and documentation comply with Joint Commission standards.
• Participate in office‑based meetings and administrative work.
