Job summary

BrightStar Care of Chicago is seeking an experienced RN Case Manager. This field-based role conducts patient assessments in clients’ homes across Chicago, manages a busy caseload, and serves as a clinical resource for field staff.

Qualifications

• Active IL RN license is required.
• Critical care experience is required.
• Reliable personal vehicle and valid driver's license with auto insurance.
• Comfortable with extensive daily driving across the Chicago area.
• Current TB test and physical.
• CPR certification is required.
• Strong assessment and clinical decision-making skills.
• Excellent communication and ability to serve as a clinical resource.

Responsibilities

• Conduct comprehensive new patient assessments in clients' homes.
• Perform scheduled and as-needed re-assessments to evaluate changes in patient condition.
• Manage an active caseload and coordinate care plans and documentation.
• Serve as a clinical resource for field nurses and guide on complex cases.
• Communicate with physicians, families, and interdisciplinary teams about patient status and care plan changes.
• Ensure care plans and documentation meet regulatory and Joint Commission standards.
• Identify and elevate clinical concerns as needed.
• Participate in weekly office meetings and case conferences (about 1 day per week).
