Job summary

FMOL Health in Louisiana is seeking a Registered Nurse to provide safe, holistic care and education to patients and families within the licensure scope. You will integrate knowledge and experience to meet patient needs across the continuum and actively participate in discharge planning and care coordination.

The role requires current Louisiana RN licensure, BLS certification, and strong communication as you collaborate with care teams and honor patient rights while advocating for quality

Qualifications

• Graduate of an accredited school of nursing.
• Proficient in English, both verbal and written.
• Current Louisiana RN license and BLS certification.

Responsibilities

• Clinical practice and care management.
• Performs psychosocial/physiologic assessments that integrates changing data.
• Formulates a plan of care based on trends of similar patients, including patient education and discharge planning.
• Tailors and prioritizes caring practice to individual needs, including cultural/ethical/spiritual needs.
• Responds to changes in patient's needs and provides holistic care using independent clinical judgment.
• Evaluates patient outcomes and revises the plan of care.
• Documents all patient care activities per documentation standards.
