Job summary

Anchorage Neighborhood Health Center seeks an RN Care Manager for HIV care and prevention to coordinate care, support adherence, and connect patients with needed services in Anchorage. You will work across primary care, behavioral health, and specialty teams to promote engagement and viral suppression.

The role requires four years of nursing experience in relevant settings, an Alaska RN license, and BLS certification.

Qualifications

• Four years of progressively responsible nursing experience in primary care, ambulatory care, community health, or population health setting.
• Active RN license in Alaska with current BLS certification.
• Bilingual skills preferred and encouraged.

Responsibilities

• Identify and engage patients who may benefit from HIV treatment and prevention case management services.
• Develop individualized, patient-centered care plans in collaboration with patients and care teams.
• Coordinate care across primary care, behavioral health, infectious disease specialists, and community partners.
• Monitor adherence to HIV treatment and prevention plans and escalate gaps as needed.
• Document patient interactions and care plans in the electronic health record.
