Job summary

Greater Lawrence Family Health Center, Inc. is seeking an RN Care Manager to enhance care for patients with HIV, viral hepatitis and substance use disorders.

The role focuses on organizing care delivery, patient follow-up, education, and coordination within the multidisciplinary team. You will lead clinic operations, manage care plans, document in the EMR, and ensure adherence to quality standards, while providing direct patient care when indicated.

Qualifications

• Gathers intake on new patients and builds a culturally appropriate plan of care.
• Provides HIV and Hepatitis education and adherence support.
• Leads HIV and Viral Hepatitis clinics with pre-visit planning and coordination.
• Documents contacts in EMR according to protocol.
• Uses physician-approved protocols to manage complex patient needs.
• Provides direct patient care when needed (immunizations, phlebotomy, DOT).
• Participates in partner notification and family education as requested.
• Assesses progress toward goals and helps patients overcome barriers to care.

Responsibilities

• Coordinate care with PCP, HIV/HCV specialists, CHW, nutritionist and pharmacists.
• Outreach to new patients to facilitate engagement in care.
• Track progress via clinical data trends and update client lists.
• Prepare HIV/hepatitis case conference documentation and participate in discussions.
• Coordinate transitions (intake, discharge, hospitalizations) for clients.
