Job summary

BBH Burrell Behavioral Health in Independence, MO seeks a Manager, HealthCare Home to oversee the daily operations of the Healthcare Home Team, coordinate clients, and lead Nurse Care Managers in collaboration and excellence. You will facilitate health education groups, ensure timely attestations and HIT tool management, and partner with other service areas to meet client needs.

A current RN license and supervisory experience are required.

Qualifications

• Active RN license in the state of practice.
• Supervisory or team-lead experience.
• Excellent verbal and written communication skills.
• Proficient computer and administrative skills.
• Ability to pass criminal background and child abuse/neglect checks.
• Complete orientation within 30 days and required trainings (Relias Learning) within two weeks annually.

Responsibilities

• Oversee the daily operation of the Healthcare Home Team.
• Coordinate and track HCH enrollees, discharges, transfers.
• Supervise Nurse Care Managers and assign caseloads.
• Coordinate and facilitate health education groups and activities for clients, staff and community.
• Submit monthly Attestation reports in CyberAccess promptly.
• Serve as Cyber Access Practice Administrator: manage user accounts and reports.
• Coordinate management of HIT tools and initiatives (CyberAccess usage, CMT reports, hospital admission/discharges).
• Oversee and facilitate staff training on Healthcare Home.
• Participate in monthly TA calls for HCH Directors and attend state-required meetings and trainings.
• Oversee Quality Assurance and Quality Improvement for Healthcare Home.
• Partner effectively with collaborating service areas to meet client needs.
