Job summary

Burrell Behavioral Health is seeking a Nurse Care Manager (RN or LPN) in Independence, MO to support clients with complex medical needs within the Healthcare Home program. This full-time role emphasizes education, care coordination, and collaborative treatment planning.

You will work with internal and external teams to implement effective plans, monitor progress through information systems, and participate in health education initiatives. Training and a strong clinical background are essential.

Qualifications

• Licensure as LPN or RN in the practicing state.
• Experience coordinating care for clients with chronic conditions is preferred.
• Strong communication and education delivery skills are required.

Responsibilities

• Collaborate with treatment team to set healthcare goals for chronic-disease clients.
• Facilitate health education groups for patients and families.
• Manage hospital and provider admissions and discharges for Healthcare Home clients.
• Educate patients on diagnoses, treatments and medications.
• Track outcomes using healthcare information systems.
• Contribute to initial treatment plan development for enrollees.
• Develop wellness and prevention initiatives to improve care.
• Respond to emergencies and coordinate rapid assistance when needed.
• Ensure compliance with QA and utilization review processes.
