Ada S. McKinley Community Services, Inc seeks an RN Health Services Coordinator to support individuals with developmental disabilities in community living arrangements. You will assess health and safety needs and coordinate Direct Support staff on medical care and medication administration.

This full-time role requires IL RN licensure and experience with developmental disabilities. You will collaborate with PCPs, labs, pharmacies, and health providers, traveling locally as needed, with regular

Qualifications

• Requires current, valid license as a Registered Nurse in Illinois.
• Requires two years of direct nursing with Developmentally Disabled adults.
• Preferred that the individual have the requisite training and experience to qualify as a Certified Developmental Disability Registered Nurse (CDDN).
• Must have use of personal vehicle, valid driver’s license and vehicle insurance.

Responsibilities

• Provides health evaluations to persons in residential programs.
• Monitors physical exams, vision/hearing, immunizations, and treatment plans.
• Monitors health records for current files and follow-up.
• Provides medication monitoring and Nursing Services Packets.
• Trains designated staff in medication administration.
• Consults with staff on healthcare-related issues, medications, health screening, and other health matters.
• Coordinates clients’ health concerns with PCP, labs, pharmacy, and health service providers.
• Participates in admission and discharge processes.
• Follows up with hospital/rehab status before home return.
• Participates in interdisciplinary meetings.
