Qualifications

• Active RN license in state of practice.
• Experience delivering resident-centered nursing care.
• Ability to supervise nursing assistants and maintain documentation.

Responsibilities

• Provide direct nursing care, administer meds, and perform skilled procedures.
• Assist residents with ADLs, ensuring dignity, safety and comfort.
• Supervise and direct nursing staff during shifts per policy.
• Monitor residents, report changes to physicians and families.
• Develop and review individualized care plans with the team.
• Maintain timely clinical documentation per policy.
• Coordinate admissions, transfers and discharges.
• Participate in QA activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
