Responsibilities

• Assess patients' physical, psychological, and social needs and improve care experience.
• Create a seamless flow through the health system to facilitate care.
• Develop and coordinate education, resources, and activities for patients and families.
• Maintain reliable systems to document, track, and monitor patient data.
• Perform clinical nursing procedures within scope of practice.
• May perform RN duties over the phone as needed.
• Perform other duties as assigned.
