Responsibilities

• Makes rounds on patients to ensure care is delivered per the individualized plan.
• Orchestrates patient flow and bed assignments within the shift.
• Assists with emergencies, admissions, and direct care activities.
• Troubleshoots for patient safety and care issues during rounds and shifts.
• Provides first line feedback and collaborates with Case Managers on resource use.
