Job summary

Fallon Health is seeking an RN or LPN for Dartmouth to provide direct nursing care for Summit ElderCare participants in a PACE setting. The role supports aging adults in community and facility contexts, following nursing processes and age-appropriate standards.

Requires MA RN or LPN license, CPR and Alzheimer’s certification (or willingness to obtain within 60 days), and 2+ years of direct adult/elderly care. Full-time, Monday–Friday with competitive pay and benefits.

Qualifications

• RN or LPN licensed in Massachusetts.
• Direct nursing care experience with adults or elderly populations.
• CPR and Alzheimer’s certification or willingness to obtain within 60 days.

Responsibilities

• Review and implement providers' orders.
• Conduct nursing assessments per policy guidelines.
• Deliver care across settings to meet individualized needs.
• Document care thoroughly in the medical record.
• Participate in care planning and IDT processes.
