Job summary

Behavioral Health Network, Inc. seeks a registered nurse to join its PACT team in Western Massachusetts. You will provide direct and indirect nursing services, coordinate assessments, and collaborate with a multidisciplinary team to deliver person-centered care.

Under supervision of the team psychiatrist, you will manage medications, support treatment plans, complete documentation in CareLogic and Smartsheet, and participate in meetings and training. Weekend/evening shifts may be required.

Qualifications

• Bachelor's degree in a related field required.
• Current, unrestricted RN license and required nursing certifications.
• At least six months of experience in human services preferred.
• Knowledge of MAP requirements, medication management, and care coordination.
• Commitment to recovery, empowerment, and person-centered care.

Responsibilities

• Provide direct and indirect nursing services to individuals served by PACT in coordination with the multidisciplinary team.
• Assess physical-health needs, referrals to medical providers, and integrate medical and psychiatric treatment.
• Follow MAP policies and document health- and medication-related activities.
• Manage medications, medical supplies, and related clinical workflows with the team.
• Support treatment plans, health goals, and recovery objectives; report progress and needs.
• Serve as primary case manager for assigned caseload when requested; complete assessments and reviews.
• Maintain shift logs, ensure安全 handling of sharps, and report safety concerns.
• Use motivational interviewing and recovery-oriented approaches to engage individuals and support community access.
