Job summary
Vivie is seeking an RN Float to deliver skilled nursing care across home health and hospice settings in Minnesota. The role requires travel between Home Health locations as needed and offers the chance to impact patient care directly in patients' homes.
You will perform nursing interventions, coordinate with the interdisciplinary team, document in EMR, and support care planning while maintaining high standards of clinical practice. A current MN RN license and CPR are required.
Qualifications
- Current MN RN license is required.
- CPR certification preferred or obtained within 6 months.
- Ability to travel between Home Health locations as needed.
Responsibilities
- Deliver skilled nursing interventions and education in home health.
- Monitor client condition and coordinate care with the interdisciplinary team.
- Maintain comprehensive EMR documentation and supervisory visits with LPNs/HHAs.
- Float to Home Health & Hospice locations as needed for coverage.
- Collaborate with families and Vivie colleagues to ensure high-quality care.
- Perform other duties as assigned.
