Qualifications

• RN licensure required (Idaho)
• Minimum 2 years of RN experience
• Associates degree or diploma in nursing and/or BSN preferred

Responsibilities

• Collaborate with physicians and multidisciplinary team to develop and manage personalized care plans for an assigned caseload.
• Coordinate care transitions and ensure care plans are patient-centered and compliant with standards of care
