Job summary

Qualifications

• RN licensed in the state of employment.
• Three years of RN experience in acute care.
• Minimum one year home health experience preferred.

Responsibilities

• Serve as a resource for field staff and physicians.
• Facilitate calls between field staff and management.
• Work in the field with staff and patients as needed.
• Maintain relationships with referral sources and families.
• Assist with monthly meetings and Start of Care compliance.
• Ensure quality and safe delivery of home health services.
