Job summary

Wellbound-Home-Care is seeking compassionate and dedicated Registered Nurses (RNs) to join our team full-time. This role provides skilled nursing care and opens new patient cases within assisted living facilities on Long Island, NY.

Qualifications

• Must have a valid NYS RN license.
• Reliable transportation and ability to drive.
• COVID-19 vaccination required (or acceptable exemption).
• Excellent communication and documentation skills.
• Proactive and self-motivated work ethic.
• Proficient with computers; EMR and PointCare experience a plus.
• Prior CHHA or home health experience preferred but not required.

Responsibilities

• Provide skilled nursing care to residents in assisted living facilities.
• Open new patient cases and coordinate care with supervising nurse.
• Maintain accurate documentation and EMR records.
