Job summary

Liberty Home Care in Niagara Falls, NY is seeking a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor. You will assess new referrals, develop patient-centered plans of care, and supervise PCA/HHA staff.

The role runs Monday–Friday, 8:30am–4:30pm, with on-call rotation stipend; NYS RN license required, associate degree, 1 year nursing experience, home care a plus. We offer comprehensive benefits and retirement options.

Qualifications

• Home care experience is a plus.

Responsibilities

• Assess newly referred patients and health needs in home care.

• Prepare written progress documentation for continuing home health services.
• Supervise and train PCA/HHA staff.
• Assist with on-call rotation (stipend provided).

Schedule: Monday - Friday 8:30am-4:30pm (37.5 hours per week)

Duties & Responsibilities:

• Provide assessment of newly referred patients and assess health needs in home care setting
• Work with patient to create a patient centered plan of care
• Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family
• Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services
• Provide supervision and training of PCA/HHA staff
• Assist with the on-call rotation (stipend provided)

Qualifications:

• NYS Licensed Registered Professional Nurse
• Minimum of Associate's Degree required
• One year Nursing experience required
• Home care experience a plus
