Responsibilities
Salary: 115k + Sign on Bonus
As an RN Field Supervisor, you will:
- Provide and interpret patient information documents such as admission packets, plans of care, advance directives, Bill of Rights, consents, and patient education materials.
- Inform patients and families about agency services, fees, policies, and rights.
- Coordinate with referrers and discharge planners to review eligibility and develop care plans.
- Assess newly referred patients, evaluate health needs and eligibility, and obtain medical histories relevant to current conditions.
- Determine the appropriate level of care and ensure a conducive environment for home care.
- Create individualized care plans based on assessments.
- Coordinate all care and communicate findings with physicians, obtaining verbal orders as needed.
- Implement treatment plans and initiate written medical orders.
- Conduct home visits for intermittent nursing services and monitor medication regimens for interactions and side effects.
- Re-assess patient needs regularly and update care plans accordingly.
- Supervise involved disciplines, review reports, and conduct competency reviews.
- Document patient progress accurately and comprehensively.
- Familiarize with resources like respiratory and DME to provide comprehensive care.
- Refer patients to additional services or community resources as needed.
- Maintain professional knowledge, participate in patient education, and contribute to quality improvement initiatives.
- Perform annual evaluations of staff, review policies, and participate in audits.
- Handle documentation, reports, and conduct competency reviews for aides.
- Consult with the DON, report significant occurrences, and address complaints and incidents.
- Reinforce agency policies with staff and provide 24-hour on-call coverage on a rotation basis.
Qualifications
Requirements of Qualified Candidates:
- Current New York State License as a Registered Nurse
- Bilingual in English/Spanish required
- Baccalaureate Degree in Nursing or related field preferred
- At least one year of nursing care experience in an acute care facility
- Desired: One year of satisfactory home care nursing experience
- Thorough knowledge of healthcare scope of practice
- Ability to interpret safety and procedural documents
- Effective communication skills
- Proficiency in mathematical calculations related to medication and measurements
- Strong organizational, writing, and interpersonal skills
- Self-motivated and dependable
- Flexible and adaptable to situational needs
