Job summary
Lightways Hospice and Serious Illness Care is seeking an RN Field Case Manager to coordinate hospice care across Lake and Cook counties. This field role delivers skilled nursing services in home and community settings as part of the interdisciplinary team to maximize patient comfort and health.
You will conduct assessments, develop care plans, and educate patients and caregivers on care and disease progression. A Monday–Friday schedule with on-call duties supports the entire service area.
Qualifications
- Current RN license.
- Graduate of accredited nursing program.
- Minimum 2 years nursing experience; 1 year in public health, home care, or hospice preferred.
Responsibilities
- Deliver skilled nursing care and hospice services in home and community settings.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments while monitoring patient symptoms and outcomes.
- Educate patients and caregivers on care, self-management, and disease progression.
- Collaborate with the interdisciplinary team to ensure coordinated, high-quality care.
- Maintain accurate documentation and participate in on-call coverage as needed.
- Monday-Friday schedule.
- Minimum of 20 on-call hours per month (may increase), supporting entire service area.
