Job summary

Lightways Hospice and Serious Illness Care is seeking an RN Field Case Manager to coordinate hospice care across Lake and Cook counties. This field role delivers skilled nursing services in home and community settings as part of the interdisciplinary team to maximize patient comfort and health.

You will conduct assessments, develop care plans, and educate patients and caregivers on care and disease progression. A Monday–Friday schedule with on-call duties supports the entire service area.

Qualifications

• Current RN license.
• Graduate of accredited nursing program.
• Minimum 2 years nursing experience; 1 year in public health, home care, or hospice preferred.

Responsibilities

• Deliver skilled nursing care and hospice services in home and community settings.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments while monitoring patient symptoms and outcomes.
• Educate patients and caregivers on care, self-management, and disease progression.
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality care.
• Maintain accurate documentation and participate in on-call coverage as needed.
• Monday-Friday schedule.
• Minimum of 20 on-call hours per month (may increase), supporting entire service area.

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.

RN Field Case Manager (North Territory which includes Lake and Cook counties):

The Field Registered Nurse (RN) Case Manager is responsible, as a member of the interdisciplinary team, for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers consistent with Lightways Hospice and Serious Illness Care policies and procedures. The RN plans and directs services to meet the needs of individuals and families/caregivers within their homes and communities.

Education/Experience:

• Current RN license
• Graduate of accredited nursing program

• Valid driver's license and insured, reliable vehicle
