Moments Hospice in Illinois seeks an RN Case Manager to deliver compassionate in-home nursing across a diverse patient caseload. You will perform initial assessments, refine care plans, and provide hands-on nursing care focused on comfort during final days, weeks, and months.

This role involves coordinating care with families and the healthcare team, maintaining open communication, and participating in interdisciplinary meetings.

Qualifications

• Current RN license in IL or eligible to practice
• Experience in case management or home health is preferred
• Excellent communication and organizational skills
• Valid driver’s license and reliable vehicle

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
