Job summary

Lightways Hospice and Serious Illness Care in Illinois is seeking an RN Field Case Manager to coordinate hospice care in the north territory, including Lake and Cook counties. You will perform skilled nursing assessments, develop care plans, and collaborate with the interdisciplinary team in patients’ homes.

This full-time role requires an active RN license, at least 2 years of nursing experience (1 year in home health, hospice, or public health), and a valid driver’s license.

Qualifications

• Current RN license required.
• Graduate of accredited nursing program.
• Minimum of 2 years of nursing experience with at least 1 year in public health, home care, or hospice.

Responsibilities

• Deliver skilled nursing care and hospice services in home and community settings.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments while monitoring patient symptoms and outcomes.
• Educate patients and caregivers on care, self-management, and disease progression.
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality care.
• Maintain accurate documentation and participate in on-call coverage as needed.
• Monday–Friday schedule.
• Minimum of 20 on-call hours per month (may increase).

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.

RN Field Case Manager (North Territory which includes Lake and Cook counties)

The Field Registered Nurse (RN) Case Manager is responsible, as a member of the interdisciplinary team, for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers consistent with Lightways Hospice and Serious Illness Care policies and procedures. The RN plans and directs services to meet the needs of individuals and families/caregivers within their homes and communities.

Key Responsibilities

Education / Experience

• Current RN license
• Graduate of accredited nursing program
• Minimum of 2 years of nursing experience, with at least 1 year in public health, home care, or hospice preferred.
• Valid driver's license and insured, reliable vehicle
