Job summary

ProHealth Physicians, part of the Optum family of UnitedHealth Group, is seeking a Registered Nurse for the Family Medicine & Pediatrics department in Vernon, CT. This full-time role offers 40 hours weekly, with a 4 days x 10 hours schedule and Tuesdays off, located at 206 Talcottville Road.

You will provide patient care, triage, education, and coordina te services, documenting all interactions in the EHR and acting as a liaison among patients, staff, and providers.

Qualifications

• Active, unrestricted CT RN licensure.
• 1+ years of nursing experience.
• Intermediate proficiency in Microsoft Office.

Responsibilities

• Provide and document patient care such as injections and preventive screenings.
• Triage patients per protocols and scheduling guidelines.
• Assist in collecting and recording clinical information for exams.
• Educate patients about their illness and self-management.
• Process standing orders including refills and tests as per policy.
• Document all patient interactions in the EHR.
• Coordinate care with providers and other services as needed.
• Maintain communications with care team and Regional Management.
