The Grand Rehabilitation & Nursing at South Point in Island Park, NY is immediately hiring Registered Nurses to join our evening shift. This full-time position offers a sign-on bonus up to $2,500 and a pay range of $55.00 - $60.00 per hour.

We value careful clinical assessment, independent decision-making, and excellent communication with residents, families, and the care team. New grads welcome to apply and grow in a supportive LTC environment.

Qualifications

• Must have an active/unrestricted NYS RN license.
• Strong clinical assessment skills.
• Ability to make independent decisions.
• Proficient in MS Office (Excel, Word, Outlook).
• Long-Term Care experience preferred but not required.
• Excellent communication and interpersonal skills.

Responsibilities

• Uses nursing judgment and knowledge of gerontological nursing to set priorities and make decisions.
• Implements physician orders accurately and in a timely manner.
• Performs standard nursing treatments and observes residents for effects.
• Assesses and updates residents’ care plans when conditions warrant.
• Completes care plans and other documents as assigned.
• Maintains confidentiality per HIPAA and guidelines.
• Supervises, directs and/or provides resident care when assigned.
• Establishes rapport with patients and families as a facility representative.
• Provides information to patients and health care team by answering questions.
• Resolves patient needs with multidisciplinary team strategies.
• Maintains documentation of patient care services.

Registered Nurse RN Duties:

• Uses nursing judgement and knowledge of gerontological nursing to set priorities and make decisions.
• Meets nursing operational standards by contributing information to strategic plans and reviews; implementing production, productivity, quality, and customer-service standards; resolving problems; identifying system improvements.
• Implements physician orders accurately and in a timely manner.
• Performs standard nursing treatments and observes resident for desired and unwanted effects.
• Assesses & implement Residents’ care plans when conditions warrant a change as necessary.
• Completes care plans and other documents as assigned.
• Complies with the confidentiality of all resident information as per HIPAA and corporate guidelines.
• Supervises, directs and/or provides resident care when assigned.
• Establishes personal rapport with potential and actual patients and other persons as a representative of the facility.
• Provides support to patients, friends, and families.
• Provides information to patients and health care team by answering questions and requests.
• Resolves patient needs by utilizing multidisciplinary team strategies.
• Maintains documentation of patient care services by auditing patient and department records.

RN Requirements/Qualifications:

• Must have an active / unrestricted NYS Registered Nurse License
• Should have excellent clinical assessment skills
• Ability to make good independent decisions
• Computer savvy; proficient in MS Office (Excel, Word, Outlook)
• Long-Term Care Experience Preferred but not required
• Excellent communication and interpersonal skills
