Job summary

HCA Florida Bayonet Point Hospital in Hudson, FL is seeking a Registered Nurse for Admission and Discharge to ensure seamless transitions for patients entering and leaving the hospital.

You will perform assessments, coordinate admissions/discharges, educate patients and families, and ensure documentation and post-hospital follow-up are completed accurately for high-quality, safe care.

Qualifications

• ACLS certification must be obtained within 6 months of employment start date
• BLS certification must be obtained within 30 days of employment start date (RN)
• Registered Nurse Associate Degree, or Bachelor’s Degree
• No Travel Required
• 2 years experience required

Responsibilities

• Obtains a nursing assessment history and ensures proper inpatient care unit admission.
• Completes and facilitates admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
• Teaches patients/families/caregivers about medical conditions, treatment plans, medications and follow-up measures.
• Completes discharge documentation, education, and discharge care for special patient populations for core measures compliance.
• Ensures patients and families receive copies of discharge paperwork and referrals for ongoing care.
• Coordinates with House Supervisors and Nursing Administration to prioritize workload and ensure smooth transitions.
• Demonstrates time-conscious prioritization of high vs low priority activities.
