Qualifications

• Minimum of High School/GED required.
• Hospital experience preferred.

Responsibilities

• Assist RN and SW Case Managers in coordinating discharge and transition plans and perform clerical tasks.
• Assist with referral processes by collecting and distributing information for patient/family choices.
• Coordinate post-discharge placement, obtain insurance verifications and authorizations.
• Contact community resources to coordinate discharge plans and unavailable services.
• Document interventions timely in EPIC and communicate with nursing/homecare facilities.
• Attend meetings, prepare agendas/minutes and manage department correspondence.
