HCA Florida Fort Walton-Destin Hospital in Florida is seeking a Registered Nurse for Admission and Discharge to deliver patient-centered care and ensure seamless transitions for patients moving through acute care settings.

The role involves performing assessments, documenting care, educating patients and families, and coordinating with nursing leadership to prioritize discharge needs while upholding safety and quality standards.

Qualifications

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• (RN) Registered Nurse must be obtained within 30 days of employment start date
• Associate Degree, or Bachelors Degree, or Registered Nurse Diploma

Responsibilities

• Obtain nursing assessment history and ensure proper inpatient care unit admission; communicate with physicians/care team as needed.
• Complete and facilitate admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
• Educate patients/families/caregivers about medical conditions, treatment plans, medications, side effects, and follow-up measures; translate medical terminology for clarity.
• Complete extra discharge documentation, education, and discharge care for special patient populations to ensure core measures compliance.
• Provide patients/families with copies of discharge paperwork and referrals for ongoing care and educational materials.
• Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify areas of greatest need for smooth transitions.
• Show time-conscious prioritization, distinguishing between low and high-priority activities.
