Job summary

A national non-profit organization is seeking a Full-Time Registered Nurse in Weber & Davis, UT. The RN will deliver care to individuals with disabilities, manage health assessments, and create support plans. Responsibilities include monitoring health conditions and building community relationships. Ideal candidates will have a valid RN license, one year of experience, and proficiency in Microsoft Office. Qualifications

• Minimum of one year of experience providing nursing services to individuals with disabilities.
• Valid driver's license with a satisfactory driving record.
• Understanding and commitment to community-based support for persons with disabilities.

Responsibilities

• Provide treatment and health care procedures ordered by a physician.
• Ensure individuals attend medical appointments per their ISP.
• Develop and update Individual Support Plans (ISP) with an interdisciplinary team.
