Job summary

CompassCare is hiring a Director of Clinical Services – RN to lead clinical operations in a hybrid role with regular Stamford in-office time. You will oversee client assessments, care planning, and clinical education across the NY tri-state area.

The role requires a current RN license, a BSN or higher, and at least five years of nursing with leadership and care coordination experience. Strong dementia and geriatrics knowledge is preferred; supervision and quality improvement are core components.

Qualifications

• Active RN license and BSN or higher.
• Experience leading clinical teams in home-based or private duty care.
• Strong knowledge of aging, chronic illness, dementia care, and care coordination.
• Excellent communication, organization and interpersonal skills.

Responsibilities

• Provide clinical oversight for client assessments and ongoing care.
• Develop and update individualized care plans and coordinate with physicians, therapists, discharge planners, social workers, and hospice providers.
• Lead transitions between home, hospital, rehab, assisted living, skilled nursing, and hospice settings.
• Offer education and guidance to clients, families, and caregivers on care options and disease progression.
• Review clinical incidents, lead follow-up actions, and drive quality improvement.
• Develop and maintain clinical policies and support strategic planning with leadership.
