Job summary

CompassCare, LLC is seeking an experienced Registered Nurse to serve as Director of Clinical Services. This hands-on leadership role leads clinical operations, quality assurance, and program development across Fairfield County and adjacent regions.

The Director will provide clinical oversight, craft care plans, mentor staff, and partner with administration to ensure safe, high-quality, client-centered care. Hybrid work with Stamford in-office presence is required.

Qualifications

• Active RN license required.
• Minimum five years of nursing experience with at least three years in Leadership, Case Management, Hospice, or related clinical work.
• Strong knowledge of aging, dementia care, care coordination, and quality management.
• Excellent communication, organizational and interpersonal skills.

Responsibilities

• Conduct comprehensive client assessments and ongoing reassessments, evaluating physical, cognitive, emotional, and environmental factors.
• Provide clinical oversight for clients with significant changes in condition, safety concerns, injuries, hospitalization, or complex care needs.
• Develop, implement and update individualized care plans, coordinating across physicians, therapists, discharge planners, social workers and hospice providers.
• Coordinate client transitions between home, hospital, rehabilitation, assisted-living, skilled nursing, and hospice settings, including discharge planning and follow up.
• Serve as a clinical advisor to clients and families, educating on changing care needs and options.
• Lead quality reviews, audits and improvement initiatives; monitor clinical outcomes and safety events.
• Develop and maintain clinical policies, procedures, and best practice guidelines; support strategic planning.

Location: Hybrid, with regular in-office time in Stamford, CT; ideally located within approximately 20 - 25 miles of Stamford

Territory: New York City, Westchester County, and Lower Fairfield County, CT

Position Type: Full-Time

Compensation: Starting at $150,000

Travel Requirements: Must be able and willing to travel regularly throughout the assigned service territory, including New York City, Westchester County, and Lower Fairfield County, CT, as needed for client visits, staff support, meetings, and other clinical responsibilities.

CompassCare is excited to be growing its team and is now looking for an experienced Registered Nurse to serve as our Director of Clinical Services — the organization’s primary clinical authority and the trusted resource clients, families, caregivers, and leadership turn to for safe, high-quality, client centered care.

This is a hands-on leadership role spanning clinical oversight, quality assurance, care, coordination, education, and program development, based in Fairfield County with regular work across Westchester County and Manhattan.

What You’ll Do

Client Assessment & Clinical Oversight

Conduct comprehensive initial assessments and ongoing client reassessments, evaluating physical, cognitive, emotional, and environmental factors.

Provide clinical oversight for clients experiencing significant changes in condition, safety concerns, injuries, hospitalization, or complex care needs care.

Planning & Coordination

Develop, implement and update individualized care plans, and coordinate services across physicians, therapists, discharge planners, social workers, and hospice providers.

Coordinate client transitions between home, hospital, rehabilitation, assisted-living, skilled nursing, and hospice settings, including discharge planning and follow up.

Family & Caregiver Support

Serve as a clinical advisor to clients and families, providing education on changing care needs, disease progression, and care options.

Serve as the primary clinical resource for caregivers and office staff on care, documentation, dementia care, hospice care, and other clinical issues.

Quality Risk & Clinical Education

Review and investigate clinical incidents and safety events (falls, wandering, medication concerns, skin integrity, hospital transfers), and lead follow up and corrective action.

Monitor clinical outcomes, conduct quality reviews and audits and drive continuous improvement initiatives.

Partner with a talent team to build training resources and competency programs for caregivers and clinical staff.

Operations, Policy & Leadership

Provide clinical guidance to the Administrator-on-Duty team, including triage processes and escalation procedures.

Participate in clinical review and intake assessments for prospective clients, including home safety evaluations and staffing recommendations.

Develop and maintain clinical policies, procedures, and best practice guidelines; contribute to strategic planning alongside operations, client services, and executive leadership.

Required Experience

• Active Registered Nurse License
• Bachelor’s degree in nursing or related healthcare field
• Minimum five years of Nursing experience, Including at least three years in Leadership, Case Management, Hospice, GeriatricHome Care, Or Related Clinical Work.
• Strong Knowledge of Aging, Chronic Illness, Dementia Care, Care Coordination and Quality Management
• Excellent Communication, Organizational and Interpersonal Skills

Preferred Experience

• Master’s degree in nursing healthcare administration or Related Field.
• Experience in Private Duty Home Care, Concierge Care, Hospice, Geriatric Care, Management or Quality Assurance Leadership.
