Job summary

West River Health Services in southwestern North Dakota seeks a dedicated Diabetes Education Coordinator to lead our ADA-compliant program and educate patients and families on diabetes management.

The role requires an active North Dakota RN license, CDE certification (or eligibility), and at least two years of patient care experience. You will collaborate with clinicians to improve outcomes and provide staff education. This is a patient-centered, community-focused setting.

Qualifications

• Current ND RN license is required.
• CDE certification, or eligible to obtain certification.
• Minimum 2 years of patient care experience.

Responsibilities

• Assess and educate patients and families on diabetic management
• Coordinate the diabetes education program and evaluate its effectiveness
• Develop follow-up plans and referrals for diabetic patients
• Serve as a resource for staff and physicians on diabetes management
