Job summary

Avera Diabetes Services-Sioux Falls is seeking an RN to provide diabetes self-management education to patients and support the nursing staff with diabetes care. The role involves planning and evaluating education programs, and collaborating with system-wide diabetes resources.

The ideal candidate will have an active RN license and 4–6 years of disease management experience, with a passion for diabetes education and adapting to new technologies.

Qualifications

• Registered Nurse (RN) with active license in the state of practice.
• Minimum 4–6 years of experience in disease management or case management preferred.
• Experience in diabetes management is desirable.

Responsibilities

• Provide individual and group diabetes self-management education.
• Serve as a clinical resource for providers and nursing staff on diabetes care.
• Develop and evaluate training programs for clinic staff in diabetic education.
• Collaborate with diabetes management across the system to maintain expert knowledge.
