Bond Health Staffing is seeking a compassionate Registered Nurse (RN) to support individuals with IDD and severe mental illness in crisis respite, intensive case management, and community transition services in Brooklyn, NY. This full-time role promotes health, safety, independence, and successful community integration.

The RN will participate in intake screenings, conduct nursing assessments, manage medications, and collaborate with an interdisciplinary team to provide comprehensive care.

Qualifications

• Current unrestricted New York State RN license.
• Minimum of two years of nursing experience.
• Experience with IDD, behavioral health, crisis care, or community health preferred.
• Knowledge of OPWDD, OMH, and DOH regulations preferred.
• Strong assessment, crisis intervention, care coordination, communication, and documentation skills.

Responsibilities

• Screen individuals for program admission during intake.
• Conduct initial and ongoing nursing assessments.
• Oversee medication administration and monitor outcomes.
• Provide direct nursing care including wound care and chronic disease management.
• Coordinate medical and behavioral health services with case management teams.
• Maintain accurate clinical documentation and ensure infection control standards.
