Job summary

Bond Health seeks a Registered Nurse for the Intensive Mobile Treatment Team in New York. The RN will assess and monitor participants' health, collaborate with a multidisciplinary team, and support recovery goals in community settings.

Requires NY RN license, 2+ years in health/mental health, strong communication, and ability to travel. Bilingual Spanish preferred. Fingerprint clearance and other requirements may apply.

Qualifications

• RN license in New York State active and in good standing.
• Minimum 2 years as RN with healthcare or behavioral health exposure.
• 2+ years experience with individuals with mental health conditions.
• Strong written and verbal communication; ability to document clearly.
• Eligible for Medicaid/Medicare participation.
• Proficient with MS Word/Excel/PowerPoint and EHR systems.

Responsibilities

• Conduct psychiatric and physical health assessments for participants.
• Develop and coordinate care plans with the treatment team.
• Monitor health status and modify treatment plans as needed.
• Administer medications, injections, vaccines, and perform phlebotomy as ordered.
• Monitor vital signs and communicate findings to the team.
• Coordinate medical appointments with psychiatrists and physicians.
• Provide treatment, rehabilitation, and recovery support services.
• Advocate for participants and connect to community resources.
• Maintain accurate clinical documentation and progress notes.
• Participate in team meetings, supervision, and trainings.
• Provide crisis intervention on a rotating basis.
