Job summary

AHRC NYC is seeking a Registered Nurse (RN) to support the Crisis Respite/Transitional Program, Intensive Case Management, and CTI. This full-time role provides comprehensive nursing, crisis stabilization, transitional planning, and intensive care coordination for individuals with IDD and SMI, facilitating health, safety, dignity, and community integration.

Schedule is Monday–Friday 9am–5pm with two days in Brooklyn and three days traveling to sites across the five boroughs; candidate must hold

Qualifications

  • Current New York State RN license (unrestricted).
  • Minimum 2 years of nursing experience; background in IDD, behavioral health, crisis care, or community health preferred.
  • Familiarity with OPWDD, OMH, and DOH nursing regulations and program requirements.
  • Strong clinical assessment, crisis intervention, and care coordination skills.
  • Willing to travel throughout the Five Boroughs.

Responsibilities

  • Screen individuals admitted to CRTP/Transitional Respite Program as intake team member.
  • Conduct initial and ongoing nursing assessments for individuals admitted to CRTP.
  • Oversight of self-medication administration program, monitor therapeutic response, and document outcomes.
  • Provide direct nursing interventions such as wound care, diabetes management, respiratory illness support, and chronic disease oversight.
  • Monitor early signs of medical or psychiatric crisis and initiate appropriate interventions.
  • Regularly reviews medication administration records, logs documenting vital signs, glucose readings, weight, and other records.
  • Reports significant issues to clinicians and team members, including ordering medications and updating prescriptions.
  • Monitor and manage medication storage and disposal procedures in accordance with federal and state law.
  • Develop and monitor health goals reflecting current person supported status.
  • Ensure infection control per OSHA guidelines and maintain necessary medical supplies.
  • 24/7 availability via phone for consultation and basic direct care.
  • Coordinate with ICM teams to address medical and behavioral health needs in service planning.
  • Deliver health education and coaching to individuals, families, and DSPs to promote independence.
  • Facilitate hospital discharge and community reintegration through CTI.
  • Coordinate follow-up care with primary care providers, specialists, and community resources.
  • Serve as clinical liaison among providers, families, and program staff.
  • Participate in interdisciplinary treatment planning and case conferences.
  • Train and mentor direct support professionals on health-related tasks and emergency response.
  • Maintain documentation in compliance with OPWDD, OMH, and DOH regulations.
