Job summary

Richmond University Medical Center is hiring an RN Care Coordinator to ensure continuity of patient care across the Amboy Medical Practice. You will support clinical and quality goals and assess readiness to learn, barriers to care, and social determinants that impact care.

The role involves pre-visit planning, daily huddles, referrals and test tracking, acting as a resource to providers and staff for effective care coordination and patient outcomes.

Qualifications

• Ambulatory care experience preferred in a clinic or physician practice.

• Strong communication with providers and care coordination capability.

Day Shift - 7.5 Hours (United States of America) The RN Care Coordinator is responsible for the continuity of patient care across the Amboy Medical Practice and supports the clinical and quality performance goals of the practice. The RN Care Coordinator routinely assesses patients for readiness to learn, barriers to care, social determinants of health and other psychosocial behaviors that impact care. The RN Care Coordinator is a resource to providers and clinical staff and ensures that the practice understands and meets the needs of its patients through pre-visit planning, daily huddles, referral and test tracking a care coordination services.

Requirements

• NYS RN license
• Current BLS
• Minimum two or more years of ambulatory, clinic or physician practice care experience required , preferably in a recognized Patient Centered Medical Home or similarly recognized program.

Shift: Mon-Fri 8:30-4:30 eo Saturday 9-5

Salary Range: $59.67/hr - $64.22/hr (Commensurate with Experience)
