The RN Care Coordinator at LE002 Amboy Medical Practice, P.C. is responsible for the continuity of patient care across the practice and supports clinical and quality performance goals.
The role involves assessing readiness to learn, barriers to care, and social determinants that impact care, and acting as a resource to providers and staff through pre-visit planning, daily huddles, referral and test tracking.

Day Shift - 7.5 Hours (United States of America)
The RN Care Coordinator is responsible for the continuity of patient care across the Amboy Medical Practice and supports the clinical and quality performance goals of the practice.
The RN Care Coordinator routinely assesses patients for readiness to learn, barriers to care, social determinants of health and other psychosocial behaviors that impact care.
The RN Care Coordinator is a resource to providers and clinical staff and ensures that the practice understands and meets the needs of its patients through pre-visit planning, daily huddles, referral and test tracking a care coordination services.
NYS RN license Current BLS Minimum two or more years of ambulatory, clinic or physician practice care experience required, preferably in a recognized Patient Centered Medical Home or similarly recognized program.
Shift: Mon-Fri 8:30-4:30 eo Saturday 9-5
Salary Range: $59.67/hr - $64.22/hr (Commensurate with Experience)
