Job summary

Qualifications

• BSN required and CA RN licensure.
• At least 3 years of acute care experience with 1+ year in discharge planning or post-acute care coordination.
• Experience with leased bed programs and transitional care preferred.
• Knowledge of Medi-Cal or other benefit programs is a plus.

Responsibilities

• Manage patient flow from Concord and Walnut Creek facilities to post-acute settings.
• Lead timely discharge planning to reduce hospital length of stay.
• Coordinate with physicians, interdisciplinary teams, families, and community partners.
• Utilize Epic EHR to document care transitions and bed resources.
