Job summary

John Muir Health seeks an RN Case Manager to advocate for optimal utilization of resources during hospitalization and through transition back to the community. You will coordinate with the interdisciplinary team and apply care coordination, education, and utilization management principles in a hospital setting.

The role requires a BSN, CA RN license, and BLS certification, with ACM or CCM certifications strongly preferred. You will work to support patient autonomy and safe discharge planning.

Qualifications

• Bachelor's Degree from an accredited School of Nursing is required.
• RN licensed in California is required by the California Board of Nursing.
• BLS certification is required by the American Heart Association.
• ACM Accredited Case Manager certification (strongly preferred).
• CCM Certified Case Manager certification strongly preferred.

Responsibilities

• Advocate for appropriate utilization of resources for hospitalized patients.
• Coordinate care across interdisciplinary health team.
• Manage transition planning from hospital to community settings.
• Educate patients and families on care plans and resources.
• Contribute to utilization management and compliance processes.
