Job summary

LiveWell Homecare Agency, LLC is seeking a dedicated Registered Nurse to coordinate Hospice, Home Health, and Home Infusion referrals across settings. You will maintain an active presence with discharge planners, social workers, physicians, and patients to ensure timely discharges and continuity of care.

Qualifications

• Active RN licensure in the United States.
• Minimum three years nursing experience; hospice/home health preferred.
• Knowledge of hospice and palliative care; CHPN a plus.
• Strong assessment, communication, and documentation skills.
• Self-directed, flexible, team-oriented, supportive of hospice philosophy.

Responsibilities

• Assess patient needs and determine eligibility for hospice and home services.
• Communicate services effectively to healthcare providers and communities.
• Provide nursing assessments, documentation, and support within regulatory guidelines.
• Support system initiatives for seamless patient care transitions.
