Job summary

LiveWell Homecare Agency, LLC is seeking a dedicated Registered Nurse to coordinate Hospice, Home Health, and Home Infusion referrals across settings. You will maintain an active presence with discharge planners, social workers, physicians, and patients to ensure timely discharges and continuity of care.

• Active RN licensure in the United States.

Job description

The role involves accepting Hospice, Home Health, and Home Infusion referrals across various settings such as acute care, physician clinics, nursing homes, and rehabilitation centers. The RN maintains an active presence, delivers excellent customer service to discharge planners, social workers, physicians, and patients, and facilitates timely discharges to ensure continuity of care.

Qualifications and skills:

• Registered Nurse with current licensure, bachelor’s degree preferred
• Minimum three years of nursing experience, hospice/home health experience preferred
• Knowledge of hospice and palliative care, with certifications such as CHPN a plus
• Excellent assessment, communication, and documentation skills
• Self-directed, flexible, team-oriented, and supportive of hospice philosophy
