Job summary

LiveWell Homecare Agency, LLC in St. Louis, MO is seeking an RN intake coordinator to manage referrals for Hospice, Home Health, and Home Infusion across acute care, clinics, and long-term care settings.

You will engage discharge planners, social workers, physicians, and patients to promote timely discharges and continuity of care, while upholding regulatory standards and quality patient care.

Qualifications

• Active RN license in the applicable state.
• Minimum three years nursing experience; hospice and home health preferred.
• Strong assessment, communication, and documentation skills.
• Self-directed, flexible, and team-oriented.
• Certification in Hospice and Palliative Nursing preferred.
• Knowledge of current clinical guidelines and regulatory requirements.

Maintains a presence in these environments, providing excellent customer service to discharge planners, social workers, physicians, and patients. Supports timely discharges and promotes continuity of care within the healthcare system.
