Job summary

Independence Health and Rehabilitation Center in Independence, OR is seeking a compassionate Registered Nurse to join a highly collaborative care team serving patients across skilled nursing, rehab, and long‑term care as they transition from hospital to home.

You’ll provide direct patient care, develop care plans with an interdisciplinary team, monitor needs, and communicate clearly with patients, families and colleagues while maintaining accurate documentation.

Qualifications

• Active Oregon RN license.
• Strong teamwork and communication skills.
• Ability to thrive in a fast-paced environment.
• Experience in skilled nursing, rehab, or long-term care preferred.

Responsibilities

• Provide direct patient care (medications, treatments, monitoring)
• Develop care plans in partnership with an interdisciplinary team
• Assess patients and respond to changing needs
• Work closely with nurses, therapists, and providers to achieve the best outcomes
