Job summary

Spring Hill Rehabilitation & Nursing Center is seeking an RN Registered Nurse to deliver state-of-the-art short and long-term skilled nursing services. You will work closely with patients, families, and staff to provide personalized care and support in a dynamic interdisciplinary team.

The role emphasizes patient-centered care, collaboration with physicians and therapists, and participation in quality improvement initiatives within a warm, empathetic environment.

Qualifications

• Current licensure as a Registered Nurse in Pennsylvania.
• 1+ years of nursing experience, preferably in a rehabilitation or long-term care setting.
• Strong communication and interpersonal skills.
• Ability to adapt to changing priorities in a fast-paced environment.

Responsibilities

• Provide high-quality, patient-centered care to residents, including assessing, planning, implementing, and evaluating individualized care plans.
• Develop and maintain effective relationships with patients, families, and interdisciplinary teams to ensure seamless care coordination.
• Collaborate with physicians, therapists, and other healthcare professionals to deliver comprehensive care.
• Monitor and report patient progress, identifying areas for improvement and implementing changes as needed.
• Maintains accurate and detailed records of patient care, treatment plans, and medication administration.
• Participates in quality improvement initiatives to enhance patient outcomes and reduce readmissions.
• Supports a positive and respectful environment, promoting a culture of empathy, inclusivity, and teamwork.
