Job summary

Elm Wood Center in New Hampshire is seeking Registered Nurses (RNs) to join our compassionate team. Full-time, part-time, and per diem positions are available, with shifts 7am-3pm, 3pm-11pm, and 7am-7pm.

Under the direction of the Nurse Supervisor, you will provide comprehensive nursing care, collaborate with the interdisciplinary team, and participate in ongoing training and mentorship through our Leading With Heart program.

Qualifications

• Education: ADN or BSN from an accredited program.
• Licensure: RN license in good standing in the state.
• Certifications: BLS/CPR.
• Clinical experience in long-term/post-acute/geriatric care preferred.
• Language: proficient in English for documentation.
• Health: must meet health screening requirements.

Responsibilities

• Deliver comprehensive nursing care and assessments.
• Develop and update care plans with interdisciplinary team.
• Mentor and supervise LPNs and CNAs as applicable.
• Ensure accurate patient documentation and billing compliance.
• Support safe discharge planning and care transitions.
