Job summary
Option Care Health is seeking a Clinical Transition Specialist, an experienced sales professional who educates patients, families and facility staff about home infusion services and products. You will help place patients in alternative site care and build strong referral relationships to grow patient starts and meet sales goals.

- Licensed RN or Pharmacist in the practicing state.
- 2+ years in healthcare industry.
- Strong relationship-building and customer service skills.
Responsibilities
- Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
- Educates patients, families and facility staff about Option Care services.
- Collaborates with Account Manager and Regional Sales Director on area business plans.
