Job summary

Option Care Health is seeking a Clinical Transition Specialist to educate patients and families about home care services and to place patients in the appropriate alternative site. You will build strong relationships with referral sources while supporting discharge planning and coordinating care transitions.

The role demands RN or Pharmacist licensure with healthcare experience, and a focus on growing referrals and achieving sales goals in a clinical setting.

Qualifications

• Licensed RN or licensed Pharmacist in the state of practice.
• Minimum 2 years in healthcare.
• Experience in patient education and care transitions preferred.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Educates patients, families and facility staff about services and products offered by Option Care.
• Provides hands-on, in-person education to patients and caregivers to support timely discharge and independence in therapy.
• Coordinates discharge processes with medical teams, case managers and intake staff to ensure seamless patient transitions.
